Who is care at home for?
It's for people assessed as needing support to live at home including:
- older people
- people with a disability
- people with a mental illness
- people with a learning disability
- people with a sensory impairment
- people caring for someone who is older or disabled
- families with young children having temporary difficulties
How to apply for care at home
Ask Social Services to visit you to assess how your care needs and explore how these can be met. We have limited funds and must make sure help is provided to the people who need it most.
How much does it cost?
We make a financial assessment of your circumstances to decide how much you pay.
The charges are reviewed each year.
How care is provided
Care in your home may be provided by:
- Social Services directly through their Home Care Service
- other agencies working on behalf of Social Services to the same standards
We can send you a weekly programme of care detailing:
- the agency providing the service
- the name(s) of the staff who will provide the care
- the planned arrival time(s)
- which task(s) you will receive assistance with.
This is called a 'Care Plan'.
The home care assistant will usually arrive within 15 minutes of the time specified.
Sometimes it may be necessary to change the time of a visit or ask a different person to call. Whenever possible you will be given 24 hours notice of any changes.
If you need to make a change to your care plan please let the care agency office know as soon as you can and at least 24 hours in advance of the planned visit.
Changes in Your Needs
If your situation changes contact Social Services so that a new assessment of your needs can be undertaken.
Reviewing your needs
At least once a year Social Services will meet with you and your carer/s to review your needs.
