We are a team of social workers, community nurses, occupational therapists and physiotherapists. We work together with other agencies to provide services to adults with a learning disability, their carers and families.
What we do
We work closely with you and other staff in the learning disability services to:
- assess your needs by meeting with you to discuss your situation
- decide what services are appropriate to meet those needs
- provide advice, information and guidance
- promote a healthy lifestyle
- run an Adult Placement scheme arranging activities, a sitting service and short stay or permanent accommodation.
How we help
Here are some of the ways we may be able to offer support, depending on the assessed needs:
- practical advice and support in your home
- support to move towards more independent living
- support for carers
- accessing day time activities
- arranging short breaks
- liaising with Community Employment Services which helps people with a disability find supported employment
- longer term living options in a variety of settings.
We will treat you fairly and with respect.
We work closely with Dorset HealthCare NHS Trust including:
- community nurses

- psychology services
- consultant psychiatrist
- physiotherapists
- speech and language therapists
- residential health units
- continence advisors
- dietician.
We also work closely with independent and voluntary organisations to provide:
- day care activities
- accommodation with support
- short breaks
- advocacy services
- advice on welfare benefits.
Will I have to pay?
There is no charge for using the services of the Community Learning Disabilities Team.
There may be a charge if you choose to accept services to meet your needs such as day time activities, support at home or accommodation.
