What is rehabilitation?

Rehabilitation can help you return to managing on your own at home:

• after an illness or injury
• through gradual deterioration of a long term illness
• if you are able to leave a care home to return to live independently.

About our services

We provide rehabilitation services for a short time to encourage and support you as you re-gain your independence.

We assess your needs and offer active programmes which aim to:

• re-build your confidence and well-being
• enable you to live safely and independently
• develop skills such as walking, washing, dressing and cooking
• prevent you going into hospital or a care home
• enable you to leave hospital early.
