Stroke CoOrdination Team

About us

We provide a Stroke Co-ordination Service if you have been discharged from hospital after a stroke. We work with you and those involved in your care to:

  • provide practical advice and information following your stay on the stroke wards
  • work with you, your carer and family to provide support during your care and rehabilitation

Help available

We ensure you have the help you need to make choices to enable you to live as independently as possible. We:

 
  • give advice on how and where to apply for welfare benefits
  • give advice and information about stroke groups, carers groups, volunteering and how to get back to work
  • let you know about agencies to help you with tasks such as getting up, going to bed, washing, dressing and going to the toilet
  • make sure you have nutritious meals delivered at home
  • advise on care homes.
