Freestyle Cards take approximately 4-5 minutes to process and there are two types available:
YELLOW Freestyle Privilege Card for the over 60's
BLUE Freestyle Priority Card, a concessionary card for those on low income
You must supply proof that you are eligible within any of the categories.
For Priority Card (blue) the following proof is required e.g. benefits book or letter of notification, disability card, doctors letter,NUS or college card, NHS tax Exemption card,
(If you receive a Working Tax Credit or Child Tax Credit you are not automatically entitled to help with health costs, and will not receive a NHS Tax Credit Exemption card, please check with Inland Revenue),
If you are a dependent child you will need to be accompanied by a parent or Guardian.
