If you live in Bournemouth, Community Care Services may be able to help if you have a learning disability and:
need extra help to stay at home
want to learn new skills
need support to live more independently
are no longer able to live at home.
Our aim is to help you live safely and independently in your local community.
Community Care Services will also support your carers - the relatives and friends who provide care for you.
The services we offer will depend on your needs and the needs of your carer(s). You may just need advice and information and we will be able to help straight away. If you think you need services to support you we will take brief details of your enquiry and arrange for a member of staff to make an appointment with you. We can then discuss your needs to assess the best way of providing support. This is called an assessment of need. Here are some of the ways we may be able to provide support depending on your needs:
Social Work Support Our Community Learning Disabilities Team:
assess the needs of people with a learning disability and their carers
arrange care to meet their needs
provide support, information and advice.
We work closely with health services including the consultant and community nurses. You should be able to access all the services you need through the Community Learning Disabilities Team.
Support at Home
Our Community Support Team may be able to help you if you have a learning disability and live independently. We offer:
practical advice and support in your home and in the community
help to sort out money matters
help to improve your communication skills.
We work closely with other people, including housing organisations and our working hours can be flexible to meet your needs.
Day Services
Attending a day centre gives you the chance to go out and:
enjoy social and leisure activities
learn new skills
meet and talk to other people
join in cultural, art and education activities
take part in work or develop skills for work.
Attending a day centre also provides a break for carer/s.
There are a range of day service venues throughout the town and we may be able to help with transport. This will be discussed when we talk about your needs and find out how you can be supported.
Housing
People with learning disabilities live successfully in different types of housing including individual self-contained properties and supported accommodation. In Bournemouth there are a range of supported living options which offer you choice and control over your life.
If you are unable to live in your own home, even with support, you may be eligible for residential care. Residential homes provide accommodation, meals and skilled care combined with social support and leisure opportunities.
Care at Home We may be able to arrange personal and practical care at home such as help with:
getting up and going to bed
washing, dressing and toileting
cooking and eating
Short Term Breaks We can arrange short term stay care breaks to meet your assessed needs. The breaks provide accommodation meals and skilled care combined with social and leisure opportunities.
Adult Placement The Adult Placement scheme offers help and support to adults with a learning disability and the people who care for them. The scheme recruits, trains and supports carers who offer support in a variety of ways, which could include:
accomodation for a short stay
permanent accommodation
acting as a sitter
enabling the person with a learning disability to join in activities in the community.
The support can vary from a few hours a month to full time care.
Advocacy
If you have a learning disability and would like help to express your needs and wishes we may be able to arrange an independent advocate to speak up for you. SCA Advocacy Service is contracted by us to provide advocates to help you with specific issues. They can be contacted at:
