Contacting Community Care Services

You can contact us direct, or ask a relative, friend, your doctor or community nurse to contact us on your behalf. This can be: in writing, by telephone, by minicom, by fax, email or by calling in to our offices in person. If you are in hospital as a patient you can speak to our staff based there.

You may just need advice and information and we will be able to help straight away. If you think you need services to support you we will take brief details of your enquiry and arrange for a member of staff to make an appointment with you. We can then discuss your needs to assess the best way of providing support. This is called an assessment of need. Community Care Services have to work within fixed budgets but at the same time must make sure help is provided to those most in need, We will not always be able to provide services, but will offer advice and information including details of other organisations who may be able to help.
